Medical Records and Birth Certificates
To request medical records
Connecticut state law requires hospitals to retain medical records for 10 years. Records older than 10 years may no longer be available at the hospital.
Here is how to obtain medical records:
For records from Greenwich Hospital, please use the instructions below. You will need Abode Acrobat Reader installed to download the form.
Complete this Authorization for Access/Release of Information.
Once completed, you may mail, fax or hand-deliver the authorization form:

You may deliver the authorization form in person to the Medical Records office during business hours, Monday - Friday, 8 am - 4:30 pm. We are located on the hospital's main campus, on the ground floor of the Watson Pavilion, 5 Perryridge Road, Greenwich.
To request copies of X-rays or other radiological images

How long does it take to receive medical records?
Depending on the complexity and size of your request, it may take up to 30 days; however, most records requests are fulfilled within 10 days of receipt of your completed authorization form. Records will be mailed to the address you provide. Please bring a photo ID if you choose to pick-up copies of your records at Greenwich Hospital.
To request birth certificates
Birth certificates are not available through Greenwich Hospital. They can be obtained from the Greenwich Town Hall, Vital Statistics Division:
