During the discharge process Ngoc Lam, PharmD identified a discrepancy in the medication reconciliation process. The After Visit Summary instructed the patient to continue taking a critical medication that was not included in the formal discharge medication orders. Further investigation showed that the last time the medication was dispensed to the patient was in 2021. Lam notified the provider, who issued a new prescription for the medication that was dispensed to the patient that same day.
